Terms and Conditions

These Terms and Conditions (the “Terms”) govern your use of kyrevivewellnessspa.com (the “Website”) and your purchase or use of mobile IV-infusion and related wellness services (collectively, the “Services”) from KY Revive, PLLC.
IMPORTANT: The Website is a marketing and informational site. It is not a medical-record system, patient portal, or emergency-services platform. Do not submit medical, health, insurance, or other sensitive personal information through Website contact forms. Clinical intake, screening, scheduling, consents, and medical records are handled through our third-party scheduling platform, Mangomint, and the applicable clinical-care process.
1. Acceptance of Terms
By accessing or using the Website, submitting a Website inquiry, booking or requesting Services through the scheduling platform, purchasing a membership, or otherwise engaging with us, you agree to these Terms. If you do not agree, do not use the Website or purchase Services.
2. Our Services and Service Area
KY Revive, PLLC provides mobile IV-infusion and related wellness services through appropriately licensed or otherwise qualified clinical personnel, subject to clinical eligibility, availability, and applicable law. Services are offered throughout Central Kentucky.
Services may be provided at a residence, workplace, hotel, event location, or other approved location. We may decline a requested location for safety, logistical, legal, clinical, or operational reasons.

3. Booking, Arrival, Rescheduling, Cancellations, and No-Shows
Booking Requests
A Website inquiry is not an appointment and does not create a provider-patient relationship. Appointments are requested and managed through Mangomint or another booking channel we designate. An appointment is not confirmed until you receive confirmation from us or our scheduling platform.
Arrival and Late Arrival
You must be present and ready at the confirmed location at the appointment time. If you are more than 15 minutes late or unavailable, we may shorten, reschedule, or cancel the appointment if clinically or operationally necessary. Fees may apply as disclosed below.
Cancellation and Rescheduling
You may cancel or reschedule without a charge by providing at least 24 hours’ notice before the appointment. A cancellation or rescheduling request made with less notice may incur a $75 fee. We may waive a fee in our discretion.
No-Shows and Inaccessible Locations
If you are unavailable, decline to provide reasonably necessary access, give materially inaccurate location information, or otherwise do not receive the appointment at the confirmed time, it is a no-show. No-shows may be charged $100, plus any disclosed travel or event-related expense.
Clinical Ineligibility at Appointment
All Services remain subject to clinical screening and the judgment of the treating clinician. If the clinician determines that you are not eligible, we may decline, modify, or stop a Service. In that event, charges and any refund will be handled under the client intake policy.
Our Cancellations
We may reschedule or cancel an appointment because of weather, unsafe conditions, clinician availability, equipment issues, circumstances beyond our reasonable control, or other legitimate clinical or operational reasons. If we cancel, we will offer a reasonable rescheduling option or refund amounts paid for the unperformed Service.
4. Revive Society Memberships
General Terms
We offer the following recurring monthly memberships (each, a “Membership”):
Revive Society Elite
$229/month
One Signature IV monthly, complimentary local travel, priority scheduling, exclusive member pricing.
Revive Society Luxe
$329/month
One Signature IV monthly, One complimentary Boost Add-on, complimentary local travel, priority scheduling, exclusive member pricing with 15% off additional IV therapy.
The Membership-specific benefits, restrictions, and pricing displayed at enrollment are part of these Terms. Membership benefits are not insurance and are not a substitute for medical care.
Automatic Renewal and Billing
Unless you cancel as described below, your Membership automatically renews each month. You authorize KY Revive, PLLC and its payment processor to charge the payment method you provide the applicable recurring monthly Membership fee, plus applicable taxes and any separately authorized charges, until you cancel.
Kentucky Auto-Renewal Disclosure: Before you submit enrollment, we will present the recurring price, billing frequency, automatic-renewal terms, cancellation policy, and cancellation method clearly and conspicuously and close to your request for consent. Your enrollment is not complete unless you take an affirmative action indicating your agreement to the recurring Membership terms. We will provide an acknowledgment that you can retain, containing the renewal terms, cancellation policy, and cancellation instructions.
If enrollment is completed online, you may cancel online. Our online cancellation method is: A link provided with appointment information. You may also cancel by contacting: lancaster.jess@icloud.com. A cancellation request must identify the account holder and be submitted 10 days before the subscription renewal to prevent the next monthly charge. Cancellation ends future renewals; it does not ordinarily refund a charge already processed, except as required by law or as we expressly agree in writing.
Transferability
Memberships and their benefits are personal to the enrolled member and are not transferable, assignable, or shareable unless KY Revive, PLLC expressly states otherwise in writing. A transfer, if permitted, remains subject to clinical screening and all other applicable conditions.
Pauses or Freezes
A member may request a pause or freeze by Contacting: lancaster.jess@icloud.com Subject to approval, a Membership may be paused for up to 1 billing cycles in any 12-month period. During a pause, your membership benefits will expire. We may require a request to be made at least 10 days before the next billing date.
Changes to Memberships
We may modify Membership pricing or material terms on prospective notice. If a change materially affects your Membership, we will provide a clear and conspicuous notice, explain how to cancel, and make the notice available in a form you can retain before the change takes effect, as applicable.
5. Medical and Wellness Disclosures
The Website and its content are for general informational and marketing purposes. They do not provide medical advice, diagnosis, or treatment and do not establish a clinician-patient relationship.
Services are not emergency care. If you have a medical emergency, call 911 or seek immediate emergency medical care.
Services are not a substitute for a relationship with a primary-care physician or other qualified healthcare professional.
We do not represent that the Services diagnose, treat, cure, prevent, or mitigate any disease or medical condition, except to the extent a specific claim is permitted by law and supported by the applicable clinician and product labeling.
Your eligibility is determined through appropriate screening and the independent clinical judgment of the treating clinician, under the oversight structure of our medical director.
A clinician may decline, modify, defer, or stop a Service at any time based on your health history, symptoms, vital signs, medications, pregnancy status, contraindications, or other clinical considerations.
Statements about wellness, hydration, recovery, energy, or similar outcomes have not been evaluated by the U.S. Food and Drug Administration unless expressly stated otherwise. The Services are not intended to diagnose, treat, cure, or prevent disease.
6. Eligibility and Client Responsibilities
Services are available only to individuals who meet our clinical and legal eligibility requirements. Unless we expressly approve otherwise in writing, you must be at least 18 years old. A parent or legal guardian must complete all required documentation for a minor, if Services to minors are offered.
You must provide complete, accurate, and current information during clinical intake. You must disclose relevant medical conditions, medications, allergies, pregnancy or breastfeeding status, and other information requested by the clinician. You must promptly notify us if any information changes before your appointment.
Pregnancy, breastfeeding, certain medical conditions, medications, allergies, symptoms, and other contraindications may make a client ineligible for some or all Services. Final eligibility rests with the treating clinician.
7. Assumption of Risk and Clinical Consents
Before receiving Services, you must complete the clinical intake, informed-consent, and other documents required by us and the treating clinician. IV therapy and related services can involve risks, including discomfort, bruising, bleeding, infection, inflammation of a vein, allergic reaction, dizziness, fainting, fluid-related complications, and other known or unforeseen adverse effects.
By choosing to receive Services after required clinical screening and informed consent, you acknowledge that you understand and voluntarily accept the inherent risks described in your clinical consent materials, to the extent permitted by law. Nothing in these Terms waives rights or remedies that cannot lawfully be waived.
8. Payment
You agree to pay all disclosed amounts for Services, travel, Memberships, cancellation fees, no-show fees, and applicable taxes. Payments are processed by Stripe or another processor we designate. We do not store full payment-card data on the Website.
Prices may change prospectively. A price change for an existing Membership will be handled as stated in Section 4 and as required by law.
9. Website Use and Intellectual Property
The Website, including its text, graphics, logos, images, and design, is owned by or licensed to KY Revive, PLLC and protected by applicable intellectual-property laws. You may use the Website only for lawful, personal, noncommercial purposes.
You may not copy, reproduce, modify, distribute, scrape, interfere with, or attempt to gain unauthorized access to the Website or its systems without our written permission.
10. Third-Party Sites and Platforms
The Website may link to Mangomint, Stripe, social-media platforms, maps, or other third-party websites and services. Those third parties operate under their own terms and privacy policies. We are not responsible for third-party content, availability, security, or practices.
11. Disclaimer of Warranties
To the fullest extent permitted by law, the Website is provided on an “as is” and “as available” basis. We do not warrant that the Website will be uninterrupted, error-free, secure, or free from harmful components. This disclaimer does not limit any clinical duty or other obligation that cannot legally be disclaimed.
12. Limitation of Liability
To the fullest extent permitted by law, KY Revive, PLLC and its owners, directors, officers, employees, contractors, clinicians, affiliates, and agents will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages arising from or relating to the Website, a booking, a Membership, or these Terms.
To the fullest extent permitted by law, our aggregate liability for a claim arising from the Website, a booking, a Membership, or these Terms will not exceed the amount you paid to KY Revive, PLLC for the specific Service or Membership giving rise to the claim during the three months immediately preceding the event giving rise to the claim.
This section does not limit liability for gross negligence, willful misconduct, or any other liability that cannot legally be limited or excluded.
13. Indemnification
To the fullest extent permitted by law, you will indemnify and hold harmless KY Revive, PLLC and its owners, directors, officers, employees, contractors, clinicians, affiliates, and agents from claims, damages, losses, and expenses (including reasonable attorneys’ fees) arising from your unlawful Website use, your violation of these Terms, or information you knowingly provide that is false or materially incomplete. This obligation does not apply to the extent a claim results from our negligence, willful misconduct, or other conduct for which indemnification is prohibited by law.
14. Governing Law, Venue, and Dispute Resolution
These Terms are governed by Kentucky law, without regard to conflict-of-law rules.
Unless a different dispute-resolution agreement applies, any legal action must be brought exclusively in the state courts located in Franklin County, Kentucky, and each party consents to that venue and jurisdiction.
15. Changes to These Terms
We may update these Terms from time to time. We will post the updated Terms on the Website and revise the “Last Updated” date. Material changes will be communicated by Website Notice. when appropriate or required by law. Updated Terms apply prospectively after their effective date. Membership changes are also governed by Section 4.
