Privacy Policy

Effective Date: August 21, 2026
Last Updated: August 19, 2026

KY Revive, PLLC (“we,” “us,” or “our”), respects your privacy. This Privacy Policy explains how we collect, use, disclose, and protect information when you visit kyrevivewellnessspa.com (the “Website”), contact us through the Website, or otherwise interact with our marketing communications.

1. Scope: Website Data Is Different from Clinical and Booking Data

This Policy applies to information collected through the Website and our Website-related marketing communications. The Website is a marketing and informational site. It is not a patient portal, clinical-record system, or system of record.

Please do not submit protected health information (“PHI”), medical information, diagnoses, symptoms, medication information, insurance information, or other sensitive health information through a Website contact form, email link, or chat feature.

If you proceed to schedule an appointment, clinical and health information is collected and stored through Mangomint, our third-party scheduling and records platform, and through the applicable clinical-care process—not through the Website. Information you provide to Mangomint, a clinician, or another clinical-care channel may be governed by separate notices, consents, and privacy practices.

2. Information We Collect Through the Website

Depending on how you use the Website, we may collect:

Information You Provide

  • Contact-form information: your name, email address, phone number, preferred service or general inquiry, and the contents of your message.

  • Marketing-preference information: your email or mobile number, consent choices, and communication preferences.

  • Other information you choose to send us: information in an email, text, social-media message, or other communication.

We ask you not to submit health or other sensitive personal information through Website forms. If you nevertheless provide it, do not assume the Website is an appropriate or HIPAA-compliant channel for that information.

Information Collected Automatically

When you visit the Website, we or our service providers may automatically collect:

  • IP address;

  • browser type, device type, operating system, and unique device or browser identifiers;

  • pages viewed, referring and exit pages, interactions, approximate location derived from IP address, and date/time information; and

  • cookie and similar-technology data.

See our Cookie Policy for more information.

3. Information Collected Through Third-Party Booking and Payment Platforms

When you use a link from the Website to book, purchase, complete intake, receive care, or manage an appointment, you leave the Website or interact with a third-party platform.

  • Mangomint: Mangomint receives and processes scheduling, intake, client-account, and clinical/medical-record information submitted through its platform.

  • Stripe: Stripe processes payment information when used for appointment or Membership payments. We do not receive or store full payment-card numbers through the Website.

We may receive limited administrative or transactional information from these platforms, such as appointment status, service purchased, Membership status, payment confirmation, or contact information, as needed to operate our business and provide services. The particular information collected by Mangomint, Stripe, and other third parties is governed by the disclosures and terms presented through their platforms.

4. How We Use Website Information

We may use Website information to:

  • respond to inquiries and communicate with you;

  • provide Website functionality and improve Website content, performance, and security;

  • analyze Website traffic and user interactions;

  • send appointment-related communications when you have provided contact information through an appropriate channel;

  • send marketing emails or text messages when you have given the required consent;

  • detect, investigate, and prevent fraud, security incidents, or misuse; and

  • comply with legal obligations, enforce our terms, and protect our rights and the rights of others.

5. How We Share Website Information

We may share Website information with:

  • Squarespace, which hosts and supports the Website;

  • Mangomint, which provides scheduling, intake, and records-platform services;

  • Stripe, which provides payment-processing services;

  • Google Analytics, which helps us understand Website usage;

  • other vendors that provide website support, analytics, email, text-message, customer-support, security, professional, or operational services;

  • our affiliates, clinicians, and personnel as necessary for legitimate business operations and in accordance with applicable law; and

  • government authorities, law-enforcement agencies, professional advisers, or other third parties when required by law or reasonably necessary to protect rights, safety, property, or legal interests.

We may also disclose information in connection with a business transaction, such as a merger, financing, acquisition, restructuring, sale of assets, or bankruptcy.

We do not sell Website visitors’ personal information for money. We do not knowingly use Website-form data for cross-context behavioral advertising. If our advertising practices change, we will update this Policy and the Cookie Policy and provide any notices or choices required by applicable law.

6. Analytics and Advertising Technologies

We use Google Analytics to measure and understand Website traffic and performance. Google Analytics may use cookies and similar technologies to collect information about Website use. We may also use, in the future, advertising or social-media pixels, tags, and similar technologies to measure campaign performance, build audience insights, or deliver relevant advertising, subject to applicable law and the choices described in our Cookie Policy.

For choices concerning cookies and similar technologies, see our Cookie Policy

7. Email and Text Messages

Marketing Messages

By separately opting in to marketing emails or text messages, you agree to receive recurring promotional communications from KY Revive, PLLC at the email address or mobile number provided. Consent is not a condition of purchasing Services.

For marketing texts:

  • message frequency may vary;

  • message and data rates may apply;

  • you may opt out at any time by replying STOP to a text message; and

  • for help, reply HELP.

We will honor reasonable opt-out requests, although you may continue to receive transactional, appointment, safety, billing, or other non-marketing messages when permitted by law.

Appointment and Service Communications

If you provide a mobile number or email address in connection with booking or receiving Services, we may send appointment confirmations, reminders, scheduling updates, invoices, or other transactional messages through Mangomint or another service provider. The consent and opt-out disclosures presented by that platform may also apply.

8. HIPAA and Notice of Privacy Practices

The Website does not collect or store PHI. Clinical and health information is handled through Mangomint and the applicable clinical-care process. For information about privacy practices applicable to clinical records, contact MangoMint or review our Notice of Privacy Practices

9. Data Retention

We retain Website information only for as long as reasonably necessary for the purposes described in this Policy, including to respond to inquiries, maintain business records, meet legal obligations, resolve disputes, enforce agreements, and protect our systems.

Unless a longer period is required or appropriate, we retain:

  • Website inquiry and marketing-preference information for 2 years after the last interaction;

  • analytics data for 2 years; and

  • opt-out and consent records for as long as needed to demonstrate and honor your communication preferences.

Clinical records and booking-platform information are retained under our clinical-record retention practices and applicable law, not under this Website retention schedule.

10. Your Choices and Privacy Requests

You may:

  • unsubscribe from marketing emails using the unsubscribe link in the email;

  • opt out of marketing texts by replying STOP;

  • manage cookies through the methods described in our Cookie Policy; and

  • request access to, correction of, or deletion of Website personal information, subject to applicable law and our need to retain information for legitimate business, legal, security, or compliance reasons.

For requests concerning clinical, scheduling, or medical-record information, use the contact process identified in our Notice of Privacy Practices or Mangomint-related communications. We may need to verify your identity before responding to a request.

11. Kentucky Residents

Kentucky residents may contact us using the information in Section 10 with questions about this Policy or to make a request concerning personal information collected through the Website. This Policy does not limit any rights available under applicable Kentucky or federal law.

Because the Website is not intended to receive PHI, do not use a Website form to request records or discuss a health condition. Use the clinical-record contact process identified in our Notice of Privacy Practices instead.

12. Children’s Privacy

The Website is not directed to children under 13, and we do not knowingly collect personal information through the Website from children under 13. If you believe a child under 13 has provided Website information to us, contact us at [Privacy Email Address], and we will take appropriate steps to delete it.

If Services for minors are offered, a parent or legal guardian must use the clinical booking and consent process rather than the Website contact form.

13. Security

We use reasonable administrative, technical, and organizational measures designed to protect Website information. No website, transmission, or storage method is completely secure. Do not transmit sensitive or medical information through Website forms or unsecured email.

14. Changes to This Privacy Policy

We may update this Policy from time to time. We will post the updated Policy on the Website and revise the “Last Updated” date. If we make a material change, we will provide additional notice when required by law.
